Resumo

            O objetivo do presente relato foi descrever um caso de miíase cervical por larvas de Cochliomyia hominivorax em um homem de 54 anos, morador de uma propriedade rural do município de Formiga, estado de Minas Gerais, Brasil, portador de uma fistula decorrente de uma cirurgia realizada para fixação de placa e parafusos ortopédicos na coluna cervical. Após relatar dor no local, foram diagnosticadas larvas de Cochliomyia hominivorax em todos os estágios evolutivos na região da fistula cervical. O paciente foi medicado (sob prescrição médica) com ivermectina 6mg duas vezes ao dia, durante dois dias, não tendo reposta terapêutica efetiva, sendo então realizada a remoção aproximadamente 180 larvas de C. hominivorax por meio da utilização de éter e pinça anatômica. Curativos diários precedidos de limpeza local com solução antisséptica foram realizados, além de antibióticoterapia com tetraciclina 500mg, quatro vezes ao dia durante três dias. O paciente apresentou evolução satisfatória, apresentando-se assintomático, três semanas após a intervenção terapêutica. Em humanos, a higiene pessoal, bem como precauções como curativos diários e higienização local de ferimentos ou fistulas pós-cirúrgicas devem ser realizados, prevenindo assim o aparecimento de miíases provocadas por larvas de C. hominivorax.

 

SUMMARY

 

The aim of this study was to report a case of cervical myiasis caused by Cochliomyia hominivorax in a 54-year-old man, a resident of a rural property in Formiga, Minas Gerais, Brazil, who carried a fistula resulting from surgery performed for fixation of plate and orthopedic screws in the cervical spine. After reporting local pain, Cochliomyia hominivorax larvae in all evolutionary stages were diagnosed in the region of the cervical fistula. The patient was medicated (prescription) with ivermectin 6 mg twice daily for two days, with no effective therapeutic response. We then proceeded to remove approximately 180 C. hominivorax larvae using ether and anatomical forceps. Daily dressings of the site preceded by cleaning with antiseptic solution were carried out, in addition to therapy with the antibiotic tetracycline 500 mg four times daily for three days. The patient recovered satisfactorily, and was asymptomatic, three weeks after the treatment. In humans, personal hygiene, and precautions such as daily dressings and local hygiene of injury or post-surgical fistulas should be performed, thus preventing the appearance of myiasis caused by larvae of C. hominivorax
